All required forms must be submitted via the Magnus Health portal by August 1 in order to start the 2026-2027 school year. Please note: The forms must be submitted electronically through Magnus, and may not be sent via mail, fax, or email. To log in to the Magnus Health portal:

• Go to www.esj.org/magnus
• Log in using your My ESJ credentials
• Click “Submit Forms via Magnus”
• Submit the remaining forms that are indicated
